Rehabilitation Aides carry out rehabilitation programs following the assessment, program development and delegation of intervention by a therapist or nurse with the goal to rehabilitate, maintain and optimize the client's physical quality of life. The Rehabilitation Aide is supervised by the therapist or nurse for the throughout that client's course of care in the program. The Rehabilitation Aide is a member of a multidisciplinary team whose intervention services are provided in a Prairie Mountain Health (PMH) facility, the client's home or community environment. Services provided include, but are not limited to, basic personal care and rehabilitation; supports for the client that are aimed at helping the client increase their level of mobility, independence with activities of daily living, community re-integration (if applicable) and maximize the client's safety. RESPONSIBILITIES: Overview : Assists the therapist with the assessment / reassessment of clients on an ongoing basis. Performs direct and indirect client care activities as assigned by the therapist or nurse for a specific client. Completes general group exercise or mobility programs as directed by the therapist, nurse or care team manager. Administers therapeutic interventions (e.g. modalities) as prescribed by the therapist. Follows the assigned duties and tasks within their scope of practice. Communicates responses to treatment and any relevant changes to the supervising therapist and/or nurse. Performs assigned client tasks including activities to promote/maximize function, mobility, movement, communication, independence in activities of daily living and instrumental activities of daily living using appropriate techniques that provide safety for clients and staff. Provides training and assistance with in-services at the request of the supervising therapist, nurse or care team manager. Ces derniers sont âgés entre 5 et 12 ans et vivent avec un handicap. L'accompagnateur permet à ces jeunes de vivre une intégration des plus significatives dans un cadre de terrain de jeux. Il favorise, également, leur participation aux activités organisées par l'animateur du groupe du camp de jour. La création d'un lien significatif avec le jeune dès le début de l'été, mais aussi auprès des parents ou tuteurs du ou des jeunes qu'il accompagne, est importante afin de bien comprendre, connaître et pouvoir répondre à leurs besoins. L'accompagnateur doit donc communiquer quotidiennement auprès des parents ou tuteurs en début et en fin de journée afin d'assurer le meilleur suivi possible. De plus, il transmet toutes les informations pertinentes au niveau des besoins, des interventions, des accommodations avec les parents/tuteurs ainsi que la coordination à l'intervention. Il peut, selon les besoins du jeune, avoir besoin de développer en collaboration avec la coordination en intervention, des outils d'intervention. Il peut avoir des formations additionnelles pour répondre à ses besoins. De plus, il peut avoir à accompagner le jeune dans ses tâches quotidiennes telles que de l'aider à manger et dans ses soins d'hygiène. Le but ultime de l'accompagnateur est que le jeune vive une expérience riche en émotions positives, de façon inclusive et éprouve du plaisir tout au long de l'été.
