As a member of the healthcare team, the Registered Practical Nurse (RPN) - New Patient Referral provides support to the Juravinski Cancer Centre (JCC) patients, families, and members of the healthcare team. Reporting to the JCC Manager Flow and Operations, the RPN responsibilities include but are not limited to: the RPN will work within the referral office, and receives patient referrals for the JCC; evaluates and triages each referral to determine placement with the appropriate clinical team; schedules patient appointments; reviews, analyzes and documents referral information including cancer related health histories, demographic data, and clinical investigation reports. In addition, working in the clinical area with newly diagnosed oncology patients. This role will provide psychosocial support to the patients and families as they start their illness trajectory. The RPN in the clinics will be completing a physical assessment of patients inclusive of interventions such as dressing changes, assisting with biopsies and other hands on activities as required such as injections and drain care. This role will also work within the outpatient oncology clinics supporting the interprofessional team within the scope of the RPN (i.e. Health history, assessments, best possible medication history, care coordination and psychosocial support to patients and families).
