Hours of Work: Weekday: Evenings , 8hrs; as per schedule
Weekend: Evenings, 8hrs; as per schedule

Summary of Duties
To provide a full range of psychosocial assessment, intervention and care planning to patients admitted to Surgical Oncology.
Responsibilities include:
- Psychosocial assessments, counselling, transition planning, advocacy and participating as a key member of the care team
- Participate in the advancement of patient care through daily inter-professional rounds, unit-based and organizational committees, quality initiatives and practice-based research
- Active member of the social work team and participate in team meetings and SW led initiatives
Qualifications/Skills
- Masters of Social Work Required
- Member in good standing of OCSWSSW
- Minimum of two years' experience in a health care setting required
- Oncology experience preferred
- Excellent assessment and intervention skills
- Knowledge of ACP and GoC skills and framework in working with this population
- Collaborative practice competencies
- Knowledge of the health care system, its legislation and community resources
- Demonstrated problem solving and conflict management skills
- Superior facilitation skills
- Commitment to Student Education and IPE activities
- Commitment to Person Centred Care
