As the Licensed Practice Nurse you will support a regional ABI Transitional Housing Program that has been co-designed in collaboration with specialists from across VCH, including brain injury, rehabilitation, substance use, mental health and safety. This is the first program of its kind in BC - one that provides a healing, client-driven ("meeting the client where they are at" and "nothing about me without me"), transitional location for people after their acute brain injury and hospitalization. As part of an interdisciplinary team in a community program you will care for clients throughout their stay including transitions and outreach to the community. The ABI program also emphasizes a supportive and innovative environment to learn and expand your knowledge and expertise. As the Licensed Practical Nurse you are passionate about helping those most impacted by the housing and toxic drug supply crises and working within a team-based care environment that emphasizes trauma informed, harm reduction and culturally safe care. Be part of a new program that will provide hope at a critical moment. As an LPN at Vancouver Coastal Health you will: Provide care to clients and families based on predictable outcomes and the needs/outcomes can be managed by following an established plan of care or under the direction/supervision/guidance of a Registered Nurse (RN). Work closely with the RN in direct care activities such as assessing, planning, implementing, evaluating and documenting client care. Support clients and families to achieve and maintain optimal health. Qualifications Education & Experience Graduation from a recognized program for Practical Nurses. Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM). Three (3) years recent experience working as a Licensed Practical Nurse providing care in the community or acute (medical/surgical/palliative, simple wound care) care setting or an equivalent combination of training, education and experience Knowledge & Abilities Comprehensive knowledge of other health care disciplines and their role in client care. Demonstrated ability in dealing with a variety of situations and responsibilities requiring initiative, creativity and professional judgment. Knowledge and experience in providing culturally safe and competent care, specifically pertaining to Aboriginal people. Knowledge and practice within the standards dictated by the BCCNM for LPNs. Broad knowledge of working with clients and families across the life span. Broad knowledge of working with Medical, Surgical and Palliative clients. Broad knowledge of Community resources and polices. Demonstrated ability to apply decision-making and analytical skills. Demonstrated ability to work collaboratively as a member of an interprofessional team. Demonstrated ability to communicate effectively both verbally and in writing. Ton équipe La succursale est ouverte un soir par semaine. Plusieurs horaires de travail sont possibles et vont varier selon les disponibilités de l'équipe. Le poste se rapporte à la directrice ou directeur de succursale.
